{"id":388,"date":"2020-06-01T17:08:26","date_gmt":"2020-06-01T21:08:26","guid":{"rendered":"http:\/\/harmsprogram.ca\/?page_id=388"},"modified":"2020-06-08T13:15:28","modified_gmt":"2020-06-08T17:15:28","slug":"beyond-udt-other-red-flags-for-harm","status":"publish","type":"page","link":"https:\/\/harmsprogram.ca\/harms-program\/v-beyond-udt-other-red-flags-for-harm","title":{"rendered":"Appendix V: Beyond UDT: Other Red Flags for Harm"},"content":{"rendered":"<div id=\"pl-388\"  class=\"panel-layout\" ><div id=\"pg-388-0\"  class=\"panel-grid panel-has-style\" ><div class=\"emphasis panel-row-style panel-row-style-for-388-0\" ><div id=\"pgc-388-0-0\"  class=\"panel-grid-cell\" ><div id=\"panel-388-0-0-0\" class=\"so-panel widget_text panel-first-child panel-last-child\" data-index=\"0\" >\t\t\t<div class=\"textwidget\"><p><em>While UDT is one marker of risk, there are numerous other indicators suggesting that a patient is being harmed, or is at risk of harm, from opioid medications&#8230;<br \/>\n<\/em><\/p>\n<\/div>\n\t\t<\/div><\/div><\/div><\/div><div id=\"pg-388-1\"  class=\"panel-grid panel-has-style\" ><div class=\"main-body-text main-table-text panel-row-style panel-row-style-for-388-1\" ><div id=\"pgc-388-1-0\"  class=\"panel-grid-cell\" ><div id=\"panel-388-1-0-0\" class=\"so-panel widget_text panel-first-child panel-last-child\" data-index=\"1\" >\t\t\t<div class=\"textwidget\"><p>While urine drug testing is one of the most objective markers of risk, there are other factors that need to be considered when weighing the risks and benefits. <a href=\"http:\/\/harmsprogram.ca\/harms-program\/9-managing-udt-results\">Chapter 9<\/a> covers how to act in the clinical context based on the overall balance, with a focus on UDT results. This section will focus on how other observations may contribute to a patient\u2019s risk assessment.<\/p>\n<p>It is important to note that not all aberrant behaviours indicate opioid misuse. Some patients may demonstrate aberrant behaviours such as drug hoarding or escalating doses without permission due to other reasons such as inadequate pain control. If pain is adequately treated, these behaviours may subside. You don\u2019t think you have enough to justify tapering and stopping at this point, but you are worried and want to put an end to this pattern of dose escalation. What options do you have?<\/p>\n<p><em>The real concern here seems to be the pattern of dose escalation. This could be from inadequately treated pain, or alternatively from opioid abuse\/misuse. One thing we have found helpful in these particular scenarios is to use the Brief Pain Inventory as a means of measuring the patient\u2019s pain and functional impairment (if dose is escalating then typically both scores will be high). If you escalate the dose and then 2 months later the patient comes back wanting another increase, you may apply the BPI again - if the score is the same or worse then naturally it looks like opioids aren\u2019t helping. Most of us at the MFHT don\u2019t use the BPI otherwise, but once an early habit of dose escalation has been started it\u2019s helpful to have this as a means of pointing that out- by the patient\u2019s own reports - opioids don\u2019t seem to be helping and therefore further escalations are not justified.
